Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.

1 p: H9 t1 W) S1 u, P4 O# K1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
& K4 u) y* p8 u5 y& ~. ]2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
; }  Y" o: O# U! @( D0 s0 l4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
$ Z% U/ c: S; Q; s/ D6. Registrations will be accepted on a first-come, first-served basis

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
