Primary purpose: The Social Worker works in partnership with settlement workers or
, O/ Q# T% b# n) T9 {9 ocommunity cultural brokers from New Canadian Health Network partner organizations to
# _* m# Y' c; W; Y; Y6 g( `provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services. $ a; u# {7 c) P8 ]9 a {) {
The position builds working relationships with New Canadian Health Network partners’ staff, 7 ?9 R9 @) Z' T3 j: C
newcomers accessing these services and other institutions and service providers. The Social / j1 }2 _0 l0 m2 X
Worker will assist in case planning, conferencing and providing support and advocacy as
- z* G% I" G: o9 P& T* n5 F6 Aneeded. As required, the Social Worker will assist in accessing community supports for
9 x( O8 d* t) }0 t2 T# Unewcomers and their families. The Social Worker provides holistic and flexible initial 6 R/ B1 S' n1 M8 d; Z2 N
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Provide intake, needs assessment, referral, case plans and action plans to support ! i4 w- l. @- P1 g+ k# e

Work in partnership with settlement workers or cultural brokers to provide supplemental * J! L& |' ]/ I6 a& N* a
case plan support for clients who have complex needs.

Qualification

% r0 e- T. M3 V* U Relevant undergraduate degree or diploma, or equivalent combination of education and
% n0 o5 V3 F# ^# mexperience in Social Work or related human service fields.
& C1 O n$ G) i o! I( } Extensive knowledge of community resources, including in depth understanding of family
- H5 D" y* ~& _: Hsupport needs ; a3 R4 _/ ~5 G. T/ ]
Experience/facility with MS Office computer applications
% S. |% s1 d3 g4 d1 y$ V Proficiency in both official languages is an asset, as is fluency in other languages
