The position builds working relationships with New Canadian Health Network partners’ staff, 2 A& f* c1 v9 T$ r
newcomers accessing these services and other institutions and service providers. The Social & s1 _) _; P9 \* `3 M G+ V% w
Worker will assist in case planning, conferencing and providing support and advocacy as
: j; n$ Z6 O: U$ X. f; `2 a& zneeded. As required, the Social Worker will assist in accessing community supports for
0 K7 S; r S$ r8 g0 Gnewcomers and their families. The Social Worker provides holistic and flexible initial - g7 ^5 P! O7 g2 u' d; w
assessment of life circumstances, needs, goals and aspirations and adheres to the client-' w# c5 V( m! y' I9 E% a

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental 1 m' |) n6 D; j% K+ c3 E

Advocate on newcomers’ behalf to programs, institutions and service providers.

experience in Social Work or related human service fields.
6 W* n$ R/ R& R Extensive knowledge of community resources, including in depth understanding of family

! y% r; c1 ]8 M, L8 `: i1 U& y0 y4 ? Experience/facility with MS Office computer applications

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate u9 X3 r, p/ @& t

Experience: Minimum of 3 years experience working in a related field * A- M2 u' P" m* w: V% e7 h8 S# m
