Nature of Work: The Social Worker will report to the manager of mental health services. ! t" w8 M9 @% c" u- ?0 C% k
The position builds working relationships with New Canadian Health Network partners’ staff,
$ [0 s; j) V, V3 _newcomers accessing these services and other institutions and service providers. The Social
7 I/ i; f/ @- a2 oWorker will assist in case planning, conferencing and providing support and advocacy as
7 `2 i" p( N6 k- Aneeded. As required, the Social Worker will assist in accessing community supports for
0 J2 X8 f$ _2 `5 K/ T" j% Gnewcomers and their families. The Social Worker provides holistic and flexible initial
+ q1 c" K& W4 [assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Providing holistic information or advocacy services for immigrants and refugees served. ) L; n! {6 K; i: {
Developing of social networks and community supports for newcomer families.
& i( z5 y3 c( L9 c' X4 a1 i Provide intake, needs assessment, referral, case plans and action plans to support 3 j3 a, s' [' _$ p# p# e
newcomers in settling and fully participating in community. 4 @, z6 F+ D4 W
Work in partnership with settlement workers or cultural brokers to provide supplemental 1 l% R% }; t1 _# x% @8 H2 a
case plan support for clients who have complex needs.

Qualification % a! y( b9 B8 P2 O3 p. E. O
- k; B9 a) O0 i& H: e Relevant undergraduate degree or diploma, or equivalent combination of education and - f& f0 s+ `3 v1 C1 \0 i X
experience in Social Work or related human service fields.
# X2 h9 N, G4 V Extensive knowledge of community resources, including in depth understanding of family 3 S9 [1 X3 j+ l- m# C- Q( w$ ?/ ?

Experience: Minimum of 3 years experience working in a related field
