community cultural brokers from New Canadian Health Network partner organizations to 0 U y6 h6 M0 s. S- s% F
provide supplemental support to newcomers who have complex needs. # `! x, D% ^; z8 C3 G
" M$ U/ q) t/ p+ D$ m# N w1 A Nature of Work: The Social Worker will report to the manager of mental health services. 0 z. C$ ^/ U' }7 v* L u& ]
The position builds working relationships with New Canadian Health Network partners’ staff, 7 l R9 @$ `4 w
newcomers accessing these services and other institutions and service providers. The Social ( T) D$ E m" i8 P
Worker will assist in case planning, conferencing and providing support and advocacy as
- G' _6 H" ^. H1 ]) e+ Kneeded. As required, the Social Worker will assist in accessing community supports for % t# D4 }+ x. Y) q O
newcomers and their families. The Social Worker provides holistic and flexible initial
; b- I% W Y9 S! G* Lassessment of life circumstances, needs, goals and aspirations and adheres to the client-( o7 s9 ]7 D5 ^3 j

Providing holistic information or advocacy services for immigrants and refugees served. ' [7 b# J* n+ k7 l8 E1 e5 W5 [
Developing of social networks and community supports for newcomer families. * Y5 T+ T9 B$ ?4 F: n$ P: c# z: ]/ O
Provide intake, needs assessment, referral, case plans and action plans to support 7 Q% {* D v( E1 g) `
newcomers in settling and fully participating in community. # c9 E8 [0 u* H
Work in partnership with settlement workers or cultural brokers to provide supplemental

& k* K f5 j- ? Relevant undergraduate degree or diploma, or equivalent combination of education and

Extensive knowledge of community resources, including in depth understanding of family
% p7 \; m$ g, D, z2 p/ L0 h7 Zsupport needs
