The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

& {! `7 }# t" X5 BThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.% K: b/ }: C F2 t6 ~: s9 `

The Nurse will report to the Clinic Coordinator.

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

Clinical Role

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. " z: _. k `, `! {
• To develop long-term, trusting relationships with both patients and peers in providing primary care services.
) ^1 s$ o* H0 l8 L• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines., A- \$ A! y! e$ H8 K
• Ensure the maintenance of accurate records in line with practice policy.1 G# m" b+ I5 }7 l- Z
• To assist in developing and implementing patient self-management programs.- s, K" m* g% V8 P/ Z$ i! b' m- o
• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

• To offer training support for other team members and assist in their professional development.

Qualifications/Experience1 f5 X+ r0 Z) C
• Baccalaureate degree in Nursing preferred
9 |6 G, Y* g/ {! A! Z, ?, B* Q• Preference given to those with at least 5 years nursing experience in primary care setting.
) ~4 Y4 s# @7 ~" u0 j7 m# d• Current CARNA Registration
- T9 U0 x3 m o6 i; A• Current CPR – Healthcare Provider3 @# C/ U5 @9 h& \' A) A w$ l
• Must have own vehicle and valid driver’s license
