How to add my wife's name into House Title

Just get married. Does anyone know how to add my wife's name into the house Title? Do I need to go to Registry? How much will I have to pay for this services? Thank you

A Transfer of Land form must be completed and submitted for registration to Land Titles along with the associated Land Titles Fees (pdf).
