Low Cost Summer Programs Registration Day

* `- C) s7 q1 C- w, u5 r  R4 C) U7 |Target:        Low income families with children/youth aged 3-17 residing in Edmonton
' S2 O; K/ N/ B. p$ W4 C0 h% s5 T) YDate        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
0 @( o  O" h( K! W! ?# D: e2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3 C8 S' S+ U# G' }% L3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
8 Y" [7 j2 ?- n5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

9 J. }/ w1 _( p+ m8 VWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
