Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
! d9 Y- V5 n$ t! O$ nFees        Ranges from $10 to $35 per week

1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
4 e8 J6 I) D  O  c. _( Q2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
2 b: i0 N) I- f' n3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes

Fee Reduction Pass Applications

& l, }5 f6 k; R, R7 ^# LWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
