! r* t. q3 p( t/ f Primary purpose: The Social Worker works in partnership with settlement workers or
' S( Q" F4 @& icommunity cultural brokers from New Canadian Health Network partner organizations to

Nature of Work: The Social Worker will report to the manager of mental health services. ' Z" z9 H& @) m
The position builds working relationships with New Canadian Health Network partners’ staff,
. m; v1 L; ^( i9 tnewcomers accessing these services and other institutions and service providers. The Social
" U: s2 Y+ N2 |3 ^" @2 EWorker will assist in case planning, conferencing and providing support and advocacy as 6 U3 F0 l: A1 l+ B7 s6 F) d0 \
needed. As required, the Social Worker will assist in accessing community supports for % Z1 l6 ?& `3 W7 |" o; q
newcomers and their families. The Social Worker provides holistic and flexible initial 3 P. S6 M8 ^. I, U W$ f' g
assessment of life circumstances, needs, goals and aspirations and adheres to the client-0 I+ u* A8 ~. F; k$ W$ b
centered delivery model.

Developing of social networks and community supports for newcomer families.

Work in partnership with settlement workers or cultural brokers to provide supplemental 6 w* N: k) a7 Q
case plan support for clients who have complex needs. ' O& O; u! {1 h& D1 l" k
Advocate on newcomers’ behalf to programs, institutions and service providers. 8 l( K3 M; `0 u$ s% Z: g
Treat all clients with dignity and patience in order to listen deeply and give thoughtful " e3 }1 A' S3 t9 V: n+ G

experience in Social Work or related human service fields. 5 I+ x5 z5 l5 {
Extensive knowledge of community resources, including in depth understanding of family * R* L, V" g4 p) V9 D4 c+ {
support needs . A# |: G n9 K% g3 G1 W
Experience/facility with MS Office computer applications
/ N1 q( c; `" c) E$ e Proficiency in both official languages is an asset, as is fluency in other languages
