Primary purpose: The Social Worker works in partnership with settlement workers or
# [3 t7 h2 ^" k; ]" J0 wcommunity cultural brokers from New Canadian Health Network partner organizations to / B$ L: i, @. s6 w" J) Z
provide supplemental support to newcomers who have complex needs. ( o2 P, N( j6 P7 [& R' L6 C, ]
, }, G) |, j: n8 j% p- k Nature of Work: The Social Worker will report to the manager of mental health services.
% I3 W- u( K# e/ IThe position builds working relationships with New Canadian Health Network partners’ staff, ; y Q2 q* | T" M; t, V
newcomers accessing these services and other institutions and service providers. The Social & j3 j. P1 j$ s1 ]1 X
Worker will assist in case planning, conferencing and providing support and advocacy as
" _! k3 J( d. K; ~9 Hneeded. As required, the Social Worker will assist in accessing community supports for
; R) R" m! y1 anewcomers and their families. The Social Worker provides holistic and flexible initial
; h; F8 ^6 w" i; v$ k, @, k3 }7 nassessment of life circumstances, needs, goals and aspirations and adheres to the client-* z2 s: t9 j8 r8 }5 ~0 M
centered delivery model. ) w& @0 f( o3 h$ Q& i R( Y. T+ ^

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support
: t( p7 Y% Q2 Y- R* v) wnewcomers in settling and fully participating in community.
; N8 O! i2 j( n* x+ Q6 i7 Q4 ~ Work in partnership with settlement workers or cultural brokers to provide supplemental

Advocate on newcomers’ behalf to programs, institutions and service providers.

2 H4 e! q& f* T% D2 m: u Relevant undergraduate degree or diploma, or equivalent combination of education and
: J1 X2 ^# B* \. H3 e" _0 h6 W gexperience in Social Work or related human service fields.
6 I3 J* c& J! ^4 w Extensive knowledge of community resources, including in depth understanding of family
/ z6 H. }. s; @0 |. l6 [support needs $ A# }" [9 H& h( }& ^$ x
Experience/facility with MS Office computer applications
7 [, {5 t" ~& A# N! q Proficiency in both official languages is an asset, as is fluency in other languages
