Primary purpose: The Social Worker works in partnership with settlement workers or
) L! I* ?6 E1 q# \3 X# ycommunity cultural brokers from New Canadian Health Network partner organizations to
! T% i" j8 w8 _+ N* ~ ~provide supplemental support to newcomers who have complex needs. % N4 ^5 K# p2 q7 E, K
& ?6 n, g- J) b: ^/ a3 k i Nature of Work: The Social Worker will report to the manager of mental health services.
" B. R3 P1 x; c u' T' sThe position builds working relationships with New Canadian Health Network partners’ staff,
9 S3 x4 q& L0 |% |& @: lnewcomers accessing these services and other institutions and service providers. The Social - d K7 n( i& y& F p! O) w) `
Worker will assist in case planning, conferencing and providing support and advocacy as + @& {" c3 V2 O+ T, W9 x- m
needed. As required, the Social Worker will assist in accessing community supports for
) L$ @- U5 A; f! T' Gnewcomers and their families. The Social Worker provides holistic and flexible initial 6 S3 V3 M6 |" h! o% C
assessment of life circumstances, needs, goals and aspirations and adheres to the client-7 Z0 W" c( ]: J. G9 f# E

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support

7 T, W* y/ c2 Z5 p7 n Experience/facility with MS Office computer applications
" D; l+ F! G1 S1 N" k) i Proficiency in both official languages is an asset, as is fluency in other languages
