PRIMARY CARE NURSE

POSITION SUMMARY

The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.. s1 P: P/ q3 b

The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.& G" F+ n& B9 X$ n6 a* n$ u

•        To work collaboratively within a family practice environment to enhance the delivery of primary care services.  A6 K. j; {( x9 f$ z9 a! B2 d9 J  ~
•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.4 U/ e& K7 [) [8 U! c5 n; n7 |& m
•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
. n% s' E. ]2 T9 e( h# a  E•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.( [% R1 Z) L6 r/ m
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  
# b6 \( ~( h1 p5 k•        To develop long-term, trusting relationships with both patients and peers in providing primary care services.
; o" h4 d; C9 O8 _•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.- t/ _  H& j: f+ z& @% i
•        Ensure the maintenance of accurate records in line with practice policy.! Z* x4 g/ `/ R. P: O! Y
•        To assist in developing and implementing patient self-management programs.
7 {9 L% L* C; `3 b•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.
" q  F1 n3 g' X  ~% v, k•        To act as a resource to other multidisciplinary team members, physicians, and community resources.0 M9 O' c4 M. O& p# P8 M4 U
•        To offer training support for other team members and assist in their professional development.# a* W$ ~- }/ V/ L% P& }

9 B( `8 O6 ~) _7 `9 z' p•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.

•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.4 n- {& M- n6 P0 H' c4 j. S1 g( s8 j
•        To participate in the Network’s education program.

) ~: |  J/ ]5 S" V) ]8 q! _SELECTION REQUIREMENTS:) b4 a; R: i* ^. V1 Y: W

: C: W$ M, x, fQualifications/Experience9 y' _- }+ T% y  k: b: U4 y
•        Baccalaureate degree in Nursing preferred+ j2 ]: t0 A* p3 H) n$ u
•        Preference given to those with at least 5 years nursing experience in primary care setting.
) d; ?( ]) f4 l, L•        Current CARNA Registration" @- {& a# Y7 ?3 l. M7 g0 E
•        Current CPR – Healthcare Provider: a& }3 |9 B' [
•        Must have own vehicle and valid driver’s license' B7 v8 B* Z$ v9 S. B
•        Knowledge of, and ability to use, electronic medical records
