" b1 { t6 N" k( ?The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.; D" V' F/ [% c+ x6 v' i+ m! v

• To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.

Qualifications/Experience7 H* y" H* {! I K' b! P
• Baccalaureate degree in Nursing preferred3 X( c' t1 q x7 P6 N) z3 v5 D0 M
• Preference given to those with at least 5 years nursing experience in primary care setting.
' O4 I* o' Z4 y/ h# m5 a6 u• Current CARNA Registration6 k. o. K, B2 K# r, [' T! q
• Current CPR – Healthcare Provider; b- @$ e" F* O e) o7 Q9 k
• Must have own vehicle and valid driver’s license
