PRIMARY CARE NURSE

POSITION SUMMARY

- Z5 h6 r/ E) n, K7 O: T0 ^5 V: J3 ZThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

! `9 k7 u1 l3 k: g( R3 wThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

•        To work collaboratively within a family practice environment to enhance the delivery of primary care services.& v; v2 L" q2 B1 B* A) o+ k
•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.
- ?7 L' j/ r% ?/ _•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.4 g2 r3 _9 ~$ d9 U  o
•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.. t% r5 S+ s5 R! N( o
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population., X  J9 r$ `4 y9 ]5 U6 l# h6 i3 j
•        Educator, communicator, advocate, coordination/navigation, supporter

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  " f% B9 t3 Z* n4 E! A
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services. # i" e3 n4 z& y
•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.% E$ b# ^# j5 w; G" A# k
•        Ensure the maintenance of accurate records in line with practice policy.

•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.' \0 }" l  i, {
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.

•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.$ ?5 Y$ [7 i4 V
•        Requires the ability to work independently and prioritize workload.
9 D* f' T; U4 s% F, O' h: h•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.
. y7 w. h% T- V) r' ?' s•        To participate in the Network’s education program.

6 A2 v1 m. ]( l) P% CSELECTION REQUIREMENTS:

5 G. ~  ~  a1 b3 dQualifications/Experience! p) ~' c6 i9 b3 M! s
•        Baccalaureate degree in Nursing preferred
, s9 |; s( M4 ~8 m& t8 w( M•        Preference given to those with at least 5 years nursing experience in primary care setting.
) n. `- H$ ?% o0 W) j, |' [5 t•        Current CARNA Registration
( V5 _/ Q8 |0 V$ l•        Current CPR – Healthcare Provider; A- C6 K+ X( Q3 x( F' J/ n' D
•        Must have own vehicle and valid driver’s license
( c3 f, ^" Q6 w# H2 v5 N•        Knowledge of, and ability to use, electronic medical records1 v6 K; z8 n( }- Z% D! W
