“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and 4 r$ x6 x; ]. t$ E1 q5 j4 j• there have been no new symptoms or change in symptoms; and/or , f8 p# Y, a) N4 E$ w- D ]1 k• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy. 8 T; ?!
