How to Apply
1.Print the Leisure Access Program Application Form.The form is also available:
* m/ D3 z- a/ c) j7 p•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
- U3 u0 y2 x( G* j! C! A: P, L" T•By calling 780-496-4918 to have one mailed to you.
+ D, e, F+ X, C' F3 o& d•For individuals under custodial care, please also submit the address verification form.
