1.Print the Leisure Access Program Application Form.The form is also available:
•At any City of Edmonton leisure, sports & fitness participating facilities.

( M: R$ {; J2 X+ t" h- @' `7 c8 [. r Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
•For individuals under custodial care, please also submit the address verification form.
$ U+ s% v) W; h! s•For individuals under government care, please also submit the consent for release of information form.   

2.Mail your completed application to:
