Target:        Low income families with children/youth aged 3-17 residing in Edmonton
) S& M5 v. O1 V7 @( }Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
9 V( H( a7 H# p6 _" ~- C3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
5 t/ `7 c7 _6 e* g6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

; D( y( _1 A/ ~; [We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
