Date        May 14, 2005 (Saturday)
% A- X0 P. h! f( g; nTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
" i1 |4 l; r+ ^Fees        Ranges from $10 to $35 per week
       
Remarks:
( l; X7 G# O$ d! D. I, C1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
9 \; |1 m$ X! Z7 C' Q1 h. `2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
' u3 ]6 C; L+ F- g. R6 {! r) n9 s4. Information on Child’s swimming level when registering for swimming classes
& c, G$ H% k; }3 `) o" V# ~! u5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
