you have three methods to book an appointment.

1.visit www.prometric.com to schedule a test

2.call 415-4164,they may ask you some questions,such as your name, date of    birth,credit card number.

3.bring a piece of check to the following address:
8 b: R9 {. K+ C3 J. r5 b5 A   Thomson Prometric Test Center
( |$ ^, x1 b' r' ~6 f            6715-177 TH ST
you take the test there and it is not far from WEM.
