' ^% A: c X0 H- ~$ dcommunity cultural brokers from New Canadian Health Network partner organizations to

* p H" C& o% q p: H9 W Nature of Work: The Social Worker will report to the manager of mental health services. 4 S0 A J% E' L6 Q
The position builds working relationships with New Canadian Health Network partners’ staff, 2 b' ?" j* a; v8 i
newcomers accessing these services and other institutions and service providers. The Social & g) K' F0 i+ R# W4 X# N2 ]
Worker will assist in case planning, conferencing and providing support and advocacy as 1 x; C7 n/ N( t- T
needed. As required, the Social Worker will assist in accessing community supports for
4 f" b: T9 k4 y2 [3 A5 m2 [4 Knewcomers and their families. The Social Worker provides holistic and flexible initial
5 u' a5 `$ \& d' xassessment of life circumstances, needs, goals and aspirations and adheres to the client-

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support

experience in Social Work or related human service fields.
% E7 V: E; @2 F Extensive knowledge of community resources, including in depth understanding of family
) C' ]8 P4 [* ]' A6 i" Y# hsupport needs 6 s( r" K5 X. I
Experience/facility with MS Office computer applications
6 x5 E# e5 o. |& e3 p, f M/ O Proficiency in both official languages is an asset, as is fluency in other languages
7 t4 F, h$ {9 c/ k Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate $ S% M% Y' T' }- T
