provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
! T) n& c1 R3 F/ ]0 ~' fThe position builds working relationships with New Canadian Health Network partners’ staff, & k9 r* y2 o9 O$ @. O; x
newcomers accessing these services and other institutions and service providers. The Social 6 G2 y4 d- ]# {7 {
Worker will assist in case planning, conferencing and providing support and advocacy as
6 z* W; H- ^1 i/ ?needed. As required, the Social Worker will assist in accessing community supports for
/ a3 d6 F0 @' e% g ?, Unewcomers and their families. The Social Worker provides holistic and flexible initial 9 c/ |: m2 m( t
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Developing of social networks and community supports for newcomer families.
0 e3 K: ?( x7 h0 i3 J6 b Provide intake, needs assessment, referral, case plans and action plans to support \3 q$ Q8 |+ e1 I& Z0 L% T
newcomers in settling and fully participating in community. 6 W6 [2 w' G. o6 |% a0 t
Work in partnership with settlement workers or cultural brokers to provide supplemental

" V- o2 R/ L5 A2 J5 x* V Relevant undergraduate degree or diploma, or equivalent combination of education and

Extensive knowledge of community resources, including in depth understanding of family 0 X4 P: U; R; K2 m# {; r+ |

Experience/facility with MS Office computer applications
! _+ A) G) M* G4 k3 k Proficiency in both official languages is an asset, as is fluency in other languages
