, |% b3 F# T# Z2 } Primary purpose: The Social Worker works in partnership with settlement workers or ; O# I' K4 ]# k0 G' ^1 Z
community cultural brokers from New Canadian Health Network partner organizations to 8 k$ B; h7 w, a. v
provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
8 }7 y9 ^! w* X! {) l# |The position builds working relationships with New Canadian Health Network partners’ staff, # ~4 J) U k: ^! q
newcomers accessing these services and other institutions and service providers. The Social
9 W3 A9 t" L( O+ H$ H2 NWorker will assist in case planning, conferencing and providing support and advocacy as
' W- x" c# P- E R% Q( m7 Xneeded. As required, the Social Worker will assist in accessing community supports for
/ W8 y3 p( F. w2 inewcomers and their families. The Social Worker provides holistic and flexible initial 0 @* I# y' K' E! e. q% `1 ]; p
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Developing of social networks and community supports for newcomer families. , Q* y" e5 K0 |+ @- x
Provide intake, needs assessment, referral, case plans and action plans to support

Relevant undergraduate degree or diploma, or equivalent combination of education and + E- H m& W" E5 e, b& l5 v
experience in Social Work or related human service fields. & e0 k7 c+ E$ s2 [% y
Extensive knowledge of community resources, including in depth understanding of family # C6 l% }2 P) C0 v4 U6 @" a
support needs / C0 k) ]3 R3 B
Experience/facility with MS Office computer applications 7 ^, y0 p( h+ W4 Q; s Z+ W! q
Proficiency in both official languages is an asset, as is fluency in other languages
+ m* n0 k$ w1 S* @7 N" Z1 o Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
