The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

•        To work collaboratively within a family practice environment to enhance the delivery of primary care services.
- X6 W4 J6 B. d' ~•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.  }0 r; b, h- G. O: l: c0 i
•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
# A' T$ n8 l1 w: G1 o5 ]•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.
/ l- u* U7 K1 c•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  * v# p0 L& D1 e
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services. 9 R& ^. u0 Y/ g( @$ v, R! O
•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.) g: m: ?2 t4 p) F0 V
•        Ensure the maintenance of accurate records in line with practice policy.. }/ S( A. [( Y1 T
•        To assist in developing and implementing patient self-management programs.
! Q! a) P$ ^. r8 X' p$ F$ _) _8 v8 Y•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.6 P, q$ w! ^9 n6 ?# ]' ?
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.8 o( s0 f) o5 e  C8 {& E
•        To offer training support for other team members and assist in their professional development.

•        The primary care nurse should have excellent written and verbal communication skills.
$ `( z$ T2 F8 z9 D& ^5 ]•        He/she should be able to work in a team while respecting the views/opinions of others.
% ~9 }' j7 f/ }- b8 \7 \6 I•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.; J& V8 F( S7 B+ M" d3 _& q
•        Requires the ability to work independently and prioritize workload.: o/ E4 L3 C+ r& X: j- F* d
•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.

SELECTION REQUIREMENTS:& T! i* K9 u) D/ b
) O; W# U- S+ w6 |1 I5 Z9 y
Qualifications/Experience
0 U) G" J8 W' l3 y) L•        Baccalaureate degree in Nursing preferred$ r2 ?4 k* z( t* j% I( }! B
•        Preference given to those with at least 5 years nursing experience in primary care setting.7 z% _' }. y1 N5 i; F! v
•        Current CARNA Registration; U' g0 Q8 K: Y9 O7 j0 v! N
•        Current CPR – Healthcare Provider
8 A; p0 C/ d. ~•        Must have own vehicle and valid driver’s license+ i. z& W% ?: C( M
•        Knowledge of, and ability to use, electronic medical records
