The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

% D( \3 M' F- o  @( a' nThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.3 O/ j: V+ N! ^5 P

Key Responsibilities

•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.
0 c  S; J$ p) y( W" {; h6 t•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
5 k' M( L, |! G& h) b! ~•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.
5 R$ {; ~: N: x5 e  i2 w•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  9 q; D3 i3 k1 ?) L
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services.
0 r# e4 _% Q7 T; U+ k•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.: [& ^. T. L! i
•        Ensure the maintenance of accurate records in line with practice policy.

•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.
/ E- |' [, ~  T6 R2 V1 J•        To act as a resource to other multidisciplinary team members, physicians, and community resources.
9 L  @8 y7 L0 j8 T6 X% P•        To offer training support for other team members and assist in their professional development.

•        The primary care nurse should have excellent written and verbal communication skills.

•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.
4 C% p6 d7 ?* p4 \8 }$ Q•        Requires the ability to work independently and prioritize workload.- N; Q6 L/ R+ A! G( ~
•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.
* x! y4 A" U2 g7 D' Q. Z•        To participate in the Network’s education program.

Qualifications/Experience8 T6 T8 e' b, n
•        Baccalaureate degree in Nursing preferred
. x" b# e8 q+ @/ r' p•        Preference given to those with at least 5 years nursing experience in primary care setting.' r" x# x) Y0 P6 ~# s: e4 |
•        Current CARNA Registration
0 `/ I. `- _5 e# x/ S; C  Z7 Q$ K; ^•        Current CPR – Healthcare Provider1 ?) r6 O7 i* i8 W# }, d+ U
•        Must have own vehicle and valid driver’s license' Q0 k$ C- ?4 o  x
•        Knowledge of, and ability to use, electronic medical records# O  H8 o0 {6 X, f. M
