The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.. o( O- X% j3 L

• To develop long-term, trusting relationships with both patients and peers in providing primary care services.
+ a5 B: k% p: F6 Y) P: ^5 }• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.6 e F% m' T: P$ L, i- x
• Ensure the maintenance of accurate records in line with practice policy.; i _! l& {5 T9 c. L; ?5 p3 R
• To assist in developing and implementing patient self-management programs.

Qualifications/Experience
+ R* L3 N, \9 n% `• Baccalaureate degree in Nursing preferred3 v* w* I# Q; g9 z4 R# W
• Preference given to those with at least 5 years nursing experience in primary care setting.
5 F5 E# q. U0 v) k: t! F• Current CARNA Registration6 n; c! Z2 F3 @$ y& w
• Current CPR – Healthcare Provider
/ t9 b+ @$ Y; t7 y• Must have own vehicle and valid driver’s license4 J5 y, m6 j3 E
• Knowledge of, and ability to use, electronic medical records- G+ T; T; w v! K
