When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic 8 F6 V( q5 L5 {condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and4 z6 P* m2 H' _$ N
• there have been no new symptoms or change in symptoms; and/or: F3 K* Q! J" r& Z( D( r
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy. W+ e7 O7 f. l+ A; }
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
