; x6 n$ t' U9 A; T% l! ~5 Y•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
' I2 b5 D0 k# J8 @/ p) p6 ~•For individuals under custodial care, please also submit the address verification form.
1 B7 d0 `8 i7 l3 W8 u' i•For individuals under government care, please also submit the consent for release of information form.
