•At any City of Edmonton leisure, sports & fitness participating facilities.

, N; L3 r  w( O6 j, z2 g4 x Monday-Friday except holidays, 8:30am-4:30pm.
' H( q- f, A2 i) ^4 m; {•By calling 780-496-4918 to have one mailed to you.
•For individuals under custodial care, please also submit the address verification form.
4 J0 }8 B" G9 C4 Z" {$ s•For individuals under government care, please also submit the consent for release of information form.
