1.Print the Leisure Access Program Application Form.The form is also available:

•14th floor CN Tower, 10004-104 Ave.
5 D! f4 @  Q$ `4 l3 o Monday-Friday except holidays, 8:30am-4:30pm.
0 f, w' W0 e" L9 g# d9 }5 w•By calling 780-496-4918 to have one mailed to you.
& n7 |+ a: p! ?! i0 C! `•For individuals under custodial care, please also submit the address verification form.
0 G+ d/ M! D' x) X! u, N•For individuals under government care, please also submit the consent for release of information form.
