Abstract: (2) unique/new employment opportunities in the Transitions Program. As a Case Manager or Community Outreach Specialist you will support clients as they move from the street to living healthier/safer lifestyles. Program services are delivered one-to-one in the areas of life management, crises intervention & management, decision making, community resourcing and employment related services/supports. 1 ^8 J. p1 z7 Z1 Q% b$ i( x* u. t. t

Key Qualifications for both positions: post secondary degree with 2 to 4 years experience or equivalent. $ m, B# G! ^- M+ i# q5 C
