Place        ASSIST Community Services Centre (9653-105A Ave)
! o% v; n2 y1 l  A5 C$ n) kPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
$ M' _- [1 f) q# [/ r$ N+ hFees        Ranges from $10 to $35 per week

Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
4 l; H) U% J6 B2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
! c% {$ W3 H" {! |# c% c! D; L. v5. Children are not required to be present at the Registration Day

9 [2 Q* N8 ?( DFee Reduction Pass Applications

& L$ ]# t/ I: H% j' v: g6 YWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
