newcomers to strengthen and enrich their life in Canada.

Primary purpose: The Social Worker works in partnership with settlement workers or . `. ]. e" m4 k
community cultural brokers from New Canadian Health Network partner organizations to ! n5 |: I2 Z, x
provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services. % _5 A# V: S, d1 F/ d. W" b
The position builds working relationships with New Canadian Health Network partners’ staff,
p. @& o8 H( `newcomers accessing these services and other institutions and service providers. The Social
9 D- m! J, u8 V' BWorker will assist in case planning, conferencing and providing support and advocacy as ( c7 R; _7 K' [9 a0 x3 A
needed. As required, the Social Worker will assist in accessing community supports for 7 \2 o0 _. A& C6 }! I) Q
newcomers and their families. The Social Worker provides holistic and flexible initial " Z4 @1 }1 }; r- L! q7 O
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Developing of social networks and community supports for newcomer families.
5 r. Y8 c" V2 F6 Y' t' i Provide intake, needs assessment, referral, case plans and action plans to support " n8 ~# ]; ], z# ]5 _
newcomers in settling and fully participating in community.
! q" t2 }- |& U7 h8 J. y Work in partnership with settlement workers or cultural brokers to provide supplemental
* y8 J0 b/ W$ u5 D# X6 ]3 ?0 tcase plan support for clients who have complex needs.

( Q& K, W* L, \6 y5 Y Relevant undergraduate degree or diploma, or equivalent combination of education and 9 D8 w. e3 n3 f3 ^/ z) [- j
experience in Social Work or related human service fields. . H( E8 q3 k5 Y' m2 a/ N y+ _
Extensive knowledge of community resources, including in depth understanding of family 3 F8 c. g9 q7 e+ w
support needs $ G/ C3 B" A, F
Experience/facility with MS Office computer applications
3 O0 u: }8 V7 d( L Proficiency in both official languages is an asset, as is fluency in other languages
& u6 g% @7 D; u1 S Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
