You can ask the other party to waive all claims related to the event. There's no standard form. You could write one up yourself after a few Google searches to give you an idea of what you should include.
Make sure you want to be protected against a medical claim that sometimes cost could spiral out of control.

通常索要对方的所有信息.

实情经过,谁地错.如对方错,必须签字承认而他不能claim.

你要保留claim的权利(受伤或财产损伤).

对方当时索赔多少当场商量.

到修车的人那去，有一个last Release Form,真正的名字忘记了。你告诉他们你的目的，他们就知道你要的是哪一个表格了.好像是保险公司最终了结赔偿是用的。
