F& M* Y) H8 Y2 XWhen the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic 9 c5 q6 g1 O! @1 M1 H* i; X+ fcondition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and' G$ l/ r- z, C$ ^+ c3 a& b
• there have been no new symptoms or change in symptoms; and/or' p9 E# S, y& K& s
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy. 3 {( x: l& S% |0 _If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
