Please make sure that you provide:; x# u' C# U" o7 ~" M
★ 2 identical photos, (black and white or colour) produced from the same untouched negative, 2 t o" `7 z0 A# X/ g
or exposed simultaneously by a split-image or multi-lens camera;
9 I& ~- i! ?' Q9 S★the name of the photographer or the studio, the studio address and the date the photos were taken on the back of the photos.$ s) o- f8 u) i4 ?' a
The photos must:* d3 @$ c) m4 t
★show a full front view of the person's head centered in the middle of the photo;, }5 E4 j2 h7 n- P M
★have a plain white background;
8 {: o, o# A; y0 |" w; z★a plain white signature strip (no more than 10mm and no less than 6mm deep) at the bottom;% z6 y* w b! E; J8 ?4 |. I/ [2 t

★35mm x 53mm finished size;" V: T) Q- Z% ?/ T; d& g
★between 25mm and 35mm from chin to crown.

★adults and children 14 years of age and older must sign their name in black or blue ink in the white signature area below each
