When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic+ l) k3 \) U; j- t7 t6 [6 ]
condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and3 y) A: X: x6 ]2 Q" u% L
• there have been no new symptoms or change in symptoms; and/or% [1 B& b2 K" i7 A7 y; G5 P
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.2 B) b1 ~. w% R* s) Z/ L$ `8 K
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
