“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and2 Q& ~( N1 @" Z' Q4 c' X
• there have been no new symptoms or change in symptoms; and/or 1 Y. ?7 [( x" _5 u7 v. d• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.) p4 }( ]( X- g2 R4 l
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
